In the UK, it ranks as the second most prevalent bacterial STI after chlamydia, with cases rising significantly in recent years. What is Gonorrhoea?

Gonorrhoea is an STD that is sexually transferred by the bacterium Neisseria gonorrhoeae and can be found in warm, wet areas of the body. Common region is the soft mucous membrane lining the urogenital tract, rectum, pharynx, and conjunctiva. 

The infection is primarily transmitted through: 

  • Sexual intercourse 
  • Contact with contaminated sex toys 
  • From infected mother to baby during birth. 

Since this disease is easily transmissible, we recommend that people know how it is transmitted and its early signs and symptoms. 

Symptoms

Gonococcal infections have different manifestations in males and females.

Symptoms in men:
  • Discomfort in the abdomen or rectum. 
  • Pain or burning while urinating. 
  • Sore or redness, and thick, cloudy, foul-smelling fluid from the penis that could be white, yellow, or green. 
  • Lumps or a painful or enlarged testicle. 
  • Anus and rectum issues such as itching, pain, bleeding, or discomfort, and burning when passing stool. 

The symptoms of Gonorrhoea are noticeable in about 90 percent of men and develop 2-10 days after the exposure. 

Symptoms in women:
  • The signs may include abnormal vaginal discharge ranging from clear, white, or cloudy, and blood-stained discharge. 
  • Painful urination or pain during sexual intercourse. 
  • Vaginal bleeding that starts and stops and is different from the normal menstrual cycle. 
  • Discomfort in the areas of the lower abdomen or pelvis. 

In women, diagnosis of Gonorrhoea may be complicated by the fact that 50 to 70 % of women may not have any symptoms. We therefore recommend regular STI screenings for all sexually active females.

Rectal and Throat Infections

Symptoms include: 

  • Bleeding through the rectum, irritation or pain felt in the anal area, or painful bowel movements. 
  • A sore throat or swollen glands, constant tiredness (fatigue), lasting or recurring high temperatures, regular headaches, joint pains, and muscle pains.

How We Test & Diagnose Gonorrhoea

Testing for Gonorrhoea is quick, painless, and highly accurate. There are several reliable and non-invasive diagnostic methods: 

  • Urine Test: Bacterial identification is commonly used, and there is no discomfort since electrodes do not enter the urethral cavity. 
  • Swab Tests: Specimens from throat swabs, cervix swabs, rectal swabs, or urethral swabs can detect the presence of bacterial Neisseria gonorrhoeae. 

We also offer at-home STI test kits for those who prefer privacy. If your test indicates an infection, we encourage you to visit us for confirmation and personalized treatment. 

Treatment Options

Treating Gonorrhoea is generally simple with the right antibiotics. Our clinic follows the latest medical guidelines to ensure effective treatment for our patients. Due to rising antibiotic resistance, the current first-line treatment is: 

  • Ceftriaxone – A single intramuscular injection of this antibiotic is the recommended treatment for uncomplicated gonorrhoea. 

Azithromycin is no longer routinely used in combination therapy, except in specific cases where co-infection with chlamydia or other considerations apply. Our clinicians will assess your case individually and recommend the most appropriate treatment. 

To ensure the infection is completely cleared, we recommend the following: 

  • Finishing the course of antibiotics as directed. 
  • Abstaining from sexual intercourse for at least a week after treatment or until a physician clears them to resume intercourse. 
  • The partners of the last 60 days should also be given tests and treatment to reduce reinfection.

Complications of Untreated Gonorrhoea

  • Pelvic Inflammatory Disease (PID): A commonly known complication in women, PID causes chronic pelvic pain, infertility, and potentially fatal ectopic pregnancies. 
  • Epididymitis: For men, this disease may lead to inflammation, pain, and swelling of the epididymis and may further contribute to impaired fertility. 
  • Disseminated gonococcal infection (DGI): This is a condition in which bacteria enter the bloodstream and cause arthritis, skin lesions, and other ‘systemic’ infections. 
  • Prostatitis: Inflammation of the prostate gland, characterized by complications in the urinary system and sexual pathology.  
  • Pregnancy complications: Gonococcal infections during pregnancy lead to a higher risk of infection, miscarriage, premature birth, and adverse birth defects among new-borns, such as blindness and life-threatening illness.

Prevention Strategies for Gonorrhoea

Preventing Gonorrhoea begins with taking these proactive measures: 

  • Using condoms: It is highly effective in preventing the spread when used properly and consistently with all the different types of sexual practices. 
  • Getting regular screenings: It’s important to undergo regular sexually transmitted infection testing, more so for those with multiple sexual partners. 
  • Limiting sexual partners: There is less risk of exposure if few sex partners are involved. 
  • Avoid risky behaviours: Alcohol and use of drugs make people more prone to risky behaviours such as having unprotected sex. 
  • Communicating with partners: Have healthy discussions about sexual health and make sure that your partners are screened.
