Myotonic Dystrophy Type 1 (DM1) is the most common adult-onset muscular dystrophy, caused by an expansion of CTG trinucleotide repeats in the DMPK gene on chromosome 19q. This PCR-based test measures the number of CTG repeats to determine whether an individual carries the pathogenic expansion.
Who Should Consider DM1 DMPK Repeat Testing?
Individuals with myotonia (difficulty relaxing muscles after contraction), progressive muscle weakness particularly of the face, neck, and distal limbs, cataracts, cardiac conduction defects, or excessive daytime sleepiness. Family members of confirmed DM1 patients seeking predictive testing. Couples planning a family, as DM1 shows anticipation (increasing severity in successive generations).
Normal alleles have 5-34 CTG repeats. Premutation alleles (35-49 repeats) may expand in future generations. Full mutation alleles (50 or more repeats) cause disease. Congenital DM1 typically involves very large expansions (over 1000 repeats). Genetic counselling is essential before and after testing due to implications of anticipation.
