HOMA-IR is calculated from your fasting glucose and fasting insulin levels and is one of the most widely used non-invasive markers of insulin resistance. Who Should Consider the Fasting Insulin Resistance Index (HOMA-IR)?
This test may be helpful if you have a family history of type 2 diabetes, are overweight or have central (abdominal) obesity, have been told you have pre-diabetes, polycystic ovary syndrome (PCOS), non-alcoholic fatty liver disease (NAFLD/MASLD) or metabolic syndrome, or experience symptoms such as persistent fatigue after meals, sugar cravings, unexplained weight gain or skin changes such as acanthosis nigricans (dark velvety patches at the neck or armpits).
HOMA-IR is also useful for tracking your response to lifestyle changes (diet, exercise, weight loss) or to medication such as metformin or GLP-1 receptor agonists. It is not a diagnostic test on its own — clinical correlation with your full history, examination and other markers (HbA1c, fasting glucose, lipids, liver function) is essential.
