Why Screen for Diabetes and Kidney Disease?

Type 2 diabetes and chronic kidney disease (CKD) are closely interconnected conditions. Diabetic nephropathy is the leading cause of end-stage renal disease in the UK. HbA1c provides a 2-3 month average of blood glucose control, while eGFR and urine ACR detect early kidney damage-often years before symptoms appear. NICE guidelines recommend annual screening for kidney function in people with diabetes.

HbA1c detects diabetes (≥48 mmol/mol) and pre-diabetes (42-47 mmol/mol)

eGFR identifies reduced kidney function before symptoms develop

Urine ACR detects early diabetic kidney damage (microalbuminuria)

Fasting glucose helps identify impaired fasting glycaemia

Early intervention can slow progression of kidney disease by up to 50%

Results guide lifestyle changes and GP referral if clinically indicated

Who Should Take This Test?

This screening is ideal for the following groups.

Family History of Diabetes

Having a first-degree relative with type 2 diabetes increases your risk 2-6 times. The NHS recommends screening if you have a parent or sibling with diabetes.

Overweight or Obese (BMI ≥25)

Approximately 80-90% of people with type 2 diabetes are overweight. The risk increases progressively with BMI, particularly with central adiposity.

Age 40 and Over

Risk of type 2 diabetes increases significantly after age 40. The NHS Health Check programme offers diabetes screening from age 40.

South Asian, African-Caribbean, or Black African Heritage

These ethnic groups have 2-4 times higher risk of type 2 diabetes. Screening is recommended from age 25 in high-risk ethnic groups.

High Blood Pressure or Cardiovascular Disease

Hypertension is a major risk factor for both diabetes and CKD. NICE recommends kidney function testing in people with hypertension.

History of Gestational Diabetes

Women with previous gestational diabetes have a 50% lifetime risk of developing type 2 diabetes and should be screened annually.

How to Prepare for Your Appointment

A little preparation goes a long way — following these simple steps helps ensure your results are as accurate as possible.

Fast for 12 hours

Your panel includes fasting blood tests such as lipid profile and glucose — please fast for 12 hours before your appointment; only water is permitted during the fasting period.

Book a morning appointment

Most patients find fasting easiest overnight — sleep through most of the fasting window and attend first thing in the morning.

Stay well hydrated

Drink plenty of water before your visit. Being well hydrated makes the blood draw quicker and more comfortable.

Wear comfortable clothing

Choose a top with loose sleeves that roll up easily for the blood sample.

Take regular medication as normal

Continue any prescribed medication unless your doctor has advised otherwise. If you can, bring a list of what you take.

Avoid alcohol & strenuous exercise

Avoid alcohol for 24 hours and heavy exercise the day before your appointment, as both can temporarily affect your results.
